Giving infant up for adoption?

I had a preemie last year..cps got involved and took custody of her..they said I couldnt handle a sick baby with no support..i got her back...and love having her but realize I cannot take care of her anymore. My car went out. I live in a small town and with no transportation or family I have no way to even go to store and get her diapers. I love her enough, to realize she deserves stability and a happy life, I have zilch support. I tried very hard. I love her with all of my heart. How do I go about finding a couple to adopt her without an agency involved? ty please dont bash me.
