How to get adderall prescription?

I am going to a new doctor, and i have to get my first physical from him. i haven't been to the doctor in over a year to be honest, but in the past year i've also developed some kind of lack of focus in school and everyday tasks. a lack of motivation almost. i will read something and find myself having to re-read it over and over to fully understand it. i am disorganized and can't prioritize so well. i'd like to ask my doctor about prescribing me a low dose of adderall, since i've tried it once and it helped very much. what should i say to him, because i don't want to get tested for ADD (too much $) because I don't think i have ADD since I used to be efficient. I think this is a weird stage I am going through
