Should I apply to good Colleges? How can I change my ways?

I recently recieved my sat scores, and I got a 2180. I knew I was smart, but I struggle immensly in school. It's not that i don't try or study, I'm just extremely disorganized. I frequently lose the notes I take (which I have trouble reading because my handwriting is so bad), and lose other assignments and the instructions to assignments. I also struggle to focus in school, I'll frequently find myself zoning out and I'll fall behind. Should I stil apply to good colleges? Mentally I can handle the work, but my organization skills I'm afraid will hold me back from success. What can i do to become more organized and focus? I really try to keep everything in order, but it just never works out.
